Schedule appointment on-line or call the office 9am-5pm M-F to speak with someone directly. However, please download and fill out forms prior to coming to the initial office visit so that your wait time will be less. Please arrive 15 minutes prior to appointment.

Fees and Payments

For your convenience our office accepts Cash, Debit Cards, Visa, and MasterCard. Payment is expected in full at the time of service. We do not accept insurance for payment; however, we will provide you with a properly coded itemized receipt that includes the appropriate diagnostic and billing codes. Many insurance plans will cover part of the cost of office visits according to their out-of-network provider schedule and may cover some or all of the costs of laboratory testing. If you do not have insurance to cover laboratory testing, then let us know as we may have special rates for lab services. Initial office visit consultation fee is $100. The cost of the procedure is $200 insertion fee for either men or women plus the cost of the pellets. Please ask if we are currently offering any specials.

We also charge a $25 fee for any appointments not cancelled prior to 48 hours before scheduled date of service. Because of limited appointment days, we want to be able to offer those patients who are on the wait-list any openings.
